Holistic, primary care services are offered by appointment and include comprehensive physicals for men and women, diagnostic laboratory services, well-child and adult immunizations and ambulatory care services for men, women and children. Tribal Health Services provides identification and coordination of care for specialty needs.
Diabetes education and management
Community Health Nurse outreach, including home visits, transportation and health education
Nutrition and lifestyle counseling
Health Promotion/Disease Prevention, including activities for addressing special health concerns aimed at youth, adults and elders
Behavioral Health, Alcohol/Substance Abuse comprehensive assessment and screening, counseling, outreach and crisis management
Support Services, including administration, medical records management, case management and social services coordination
Contract Health Services for eligible clients, including identification and access to alternate resources and processing referrals and payments to contract health providers
