Tribal Health Services

The Tribal Health Services (THS) facility, funded by the Mashantucket Pequot Tribal Nation and the national Indian Health Services (IHS) agency and other health-related grants, is available to members of federally recognized tribes residing in our service area in addition to members of the Mashantucket Pequot Tribal Nation.

Patient Eligibility

Eligibility to use MPTN Tribal Health Services is determined by both Indian Health Services regulations and tribal resolution. One must be a member of a federally recognized Tribe demonstrated by tribal enrolled document.

Eligibility for Patient and Referred Care services has an additional set of criteria including having a socio-economic tie to MPTN and living in our service area. All registered patients are not eligible for this program.

Documents that are needed for your registration include: Copy of birth certificate, social security card, piece of mail showing address, tribal identification, copy of insurance card, copy of marriage license* and copy of drivers license/state id. Please submit these documents along with Patient Registration Application. Please contact 860-312-8025 with additional questions.

Purchased & Referred Care Program

The PRC program assists patients with payment for services not offered by our tribal health facility based on funding availability, current level of service and patient notification.  Use of alternate resources is a requirement before PRC funds are used.
